You must fill out one form for each dog. We will contact you if we are missing any information.
Please note: The dog is not officially licensed until you receive the tag and paper license in the mail from the Town Clerk.
This Online form is Only usable until April 1st of each Year. You may Print Out the Form, or visit Cole Hall to Register your Dogs during other times.
(Owner must be at least 18 years old and able to take legal responsibility for the animal.)
